At Benefit Advisors, Inc., we collect, maintain and use personal information for business purposes and take this responsibility very seriously. The following is how we at Benefit Advisors, Inc. protect the privacy, use, access and disclosure of Protected Health Information (PHI). Please review the following communication carefully.

Benefit Advisors, Inc. Safegaurds this Information

We maintain appropriate physical, electronic and procedural safeguards to maintain the confidentiality and security of your personal health information. All employees are held to strict standards of conduct in order to ensure the confidentiality of this information. Employees are continually trained on how to keep client information safe.

Benefit Advisors, Inc. Collects

We collect personal health information about you from the following sources:
  • Information received from you on applications including but not limited to: claim forms, enrollment forms, beneficiary designation forms, benefit forms or any other forms necessary to administer coverage or assist with administration of our clients' group and/or individual insurance programs.
  • We collect infomration about your transactions with our affiliates, others, and us
  • We collect information from you and others in assisting in marketing, implementing and/or administration of insurance coverage

  • Benefit Advisors, Inc. Limits Disclosing Information

    Benefit Advisors, Inc. may use or disclose your protected health information to make payment or collect payment from third parties without authorization. Furthermore, Benefit Advisors, Inc. may disclose your health information when it is required to do so by any federal, state or local law. Other than as stated above, Benefit Advisors, Inc. will not disclose your health information other than with your written authorization. If you authorize us to disclose this information, you may revoke that authorization in writing at any time.

    Your Rights to Health Information

  • You may request restrictions on uses and disclosures of your health information. For example you may request a limit on disclosure of your health information to someone involved in the payment of your care. However, Benefit Advisors, Inc. is not required to agree to this request.
  • You have the right to communicate with Benefit Advisors, Inc. in a certain way you feel confident that your health information would not be jeopardized in any way. For example by a certain telephone number or e-mail address. Benefit Advisors, Inc. will attempt to honor your reasonable requests for confidential communications.
  • You have the right to inspect your health information by sending a written communication to Benefit Advisors, Inc.. If you request a copy of your health information, Benefit Advisors, Inc. may charge a reasonable fee for copying, assembling cost, postage and any applicable fees associated with your request.
  • You have the right to amend your health information records if you believe they are incomplete or inaccurate if Benefit Advisors, Inc. maintains this information. Benefit Advisors, Inc. may deny this request if it does not include a reason to support the amendment or health information records were not created by Benefit Advisors, Inc., although this information may be inspected. The request must be made in writing to Benefit Advisors, Inc..
  • You have a right to request a list of your health information made by Benefit Advisors, Inc. for any reason other than for treatment, payment or health operations. This request must be made in writing. Accounting requests may not be made for periods of time going back more than six years. Benefit Advisors, Inc. will provide the first accounting you request during any 12-month period without charge. Subsequent requests may be subject to reasonable cost-based fee.

  • Duties of Benefit Advisors, Inc.

    We are required by law to maintain the privacy of your health information as set forth in this notice and to provide you this Notice of its duties and privacy practices. Benefit Advisors, Inc. is required to abide by the terms of this notice, which may be amended from time to time. We reserve the right to change the terms of this Notice. If the Notice is changed you will be provided a copy of the revised Notice within 60 days of the change. Furthermore, we will provide this notice upon request to anyone who requests it, even if the participant agreed to receive the Notice electronically. Finally, you have the right to express complaints to Benefit Advisors, Inc. and to the Secretary of the Department of Health and Human Services if you believe your privacy rights have been violated.
